Easy ----> Rank A, 2 credit/2P start yields 2 player game
Medium --> Rank D, 2 credit/2P start yields 1 player game with double + 1 starting lives
Hard ----> Rank D, 2 credit/2P start yields 2 player game
Hardest -> Rank A, 2 credit/2P start yields 1 player game with double + 1 starting lives
... so to play at a difficulty of "Rank D", please use either "Medium" or "Hard", but if you also want to use the Player 2 side (to display up to 8-digits in your score), you must use "Hard".
